What is Community Connect Victoria?

Community Connect Victoria is a new organisation created to deliver Support Coordination, Psychosocial Recovery Coaching and Aged Care Coordination services.

These services are transitioning from Disability Connect Victoria into a dedicated business so we can continue to grow, specialise and expand the range of supports available to our clients.

In the future, this will allow Community Connect Victoria to make new services available to existing clients, and new clients, including Behaviour Support, Group Activities and more.

Why are these services moving to a new organisation?

As our services and client base have grown, we identified the need to separate coordination and recovery-focused services from direct support delivery.

This allows each organisation to:
 • Maintain a clear service focus
 • Invest in specialist staff and systems
 • Expand service offerings without compromise
 • Improve responsiveness and outcomes for clients

This change is about improving services — not changing them.

Question 3

I'm an existing client of DCV, will my current supports change?

Accepted Answer

No.

You will continue to receive the same level of care, professionalism, and commitment that you currently experience.
 
Your supports will remain:
 • Person-centred
 • Outcome-focused
 • Aligned with your goals and funding
 • Delivered by experienced and qualified staff

The transition is administrative only and does not affect the quality or availability of your supports.

Question 4

Will I keep the same Support Coordinator or Recovery Coach?

Accepted Answer

Yes.

Wherever possible, you will continue working with the same staff, including your Support Coordinator or Psychosocial Recovery Coach.

There is no requirement to change workers unless you request a change or there is a genuine operational reason, which would always be discussed with you first.

Do I need to do anything?

Accepted Answer

No immediate action is required.

If any documentation updates are needed (such as service agreements or provider details), our team will:
 • Explain everything clearly
 • Provide support with any required signatures
 • Ensure there is no disruption to your services

We will guide you through every step.

Question 6

Will this affect my NDIS or My Aged Care funding?

Accepted Answer

No.

Your funding does not change, and your supports will continue to be delivered in line with your approved funding.

Question 7

Can I still access Disability Support Worker services from Disability Connect Victoria?

Accepted Answer

Yes.

If you currently receive Disability Support Worker services from Disability Connect Victoria, these will continue to be delivered by Disability Connect Victoria, while Community Connect Victoria focuses on coordination and recovery-based supports.

The two organisations will continue to work closely together to ensure seamless support for shared clients.

If you currently receive support worker services from another organisation, these will not be impacted by this change either.

Question 8

Will Community Connect Victoria offer new services in the future?

Accepted Answer

Yes.

Community Connect Victoria is being developed as a specialist coordination and recovery hub, with plans to introduce additional services over time, including:
 • Positive Behaviour Support
 • Group programs and group-based activities
 • Capacity-building and skill-development programs
 • Community participation initiatives

These services will be introduced gradually and in response to client needs.

Will my information still be kept private?

Accepted Answer

Absolutely.

Your privacy and confidentiality remain a priority.

All personal information will continue to be handled in line with:
 • Privacy legislation
 • NDIS Practice Standards
 • Organisational privacy and data protection policies

Information is only shared where required to support your care and with appropriate consent.

Community Connect Victoria will never share your personal information with another person or organisation without your explicit informed consent.

Also, Community Connect Victoria will never sell your personal information with any 3rd party.

Who do I contact if I have questions or concerns?

You can continue to contact your current Support Coordinator, Recovery Coach, or the admin team.

We encourage you to reach out at any time — your questions are welcome, and we are committed to transparency throughout this transition.

What does this change mean for me overall?

Accepted Answer

In short:
 • You keep the same support
 • You keep the same people
 • You gain access to more specialised services in the future

This transition is designed to strengthen services, not disrupt them, and to ensure we continue delivering high-quality, person-centred supports into the future.

Community Connect Victoria is a trading name of Disability Connect Australia and our ABN is 15 637 080 623.

Disability Connect Australia is a registered NDIS, TAC and WorkSafe service provider, and our registration extends to all of our trading business names including but not limited to Community Connect Victoria.
